Medical Staff Orientation
WELCOME TO ONLINE MEDICAL STAFF ORIENTATION
Online Medical Staff Orientation was designed with convenience in mind, and is your opportunity to complete requirements linked to the initial application process. There are five (5) elements required in order to complete the orientation process:
- View the Medical Staff Orientation video presentation in its entirety - approximately 16 minutes
- Read the CHRISTUS Physician Compact
- Review the CHRISTUS Code of Ethics
- View the Medication Ordering Policy slide presentation
- Read the National Patient Safety Goals: Target on Infections
The links above will take you to the required elements. Once you have viewed the video in its entirety, you will be able to print the Certification of Completion form. This form must be completed by indicating the dates of completion for each element. Sign and present the form to Physician Services prior to your appointment date.
PLEASE NOTE:
Failure to return the signed Orientation Certification of Completion to Physician Services will result in a delay in the appointment process, which may include being unable to complete your application.
In addition, your CHRISTUS Santa Rosa identification badge and parking tag will only be issued following receipt of the signed orientation certification form.
Once you have received notification that your privileges are approved, call 210.704.4444 and leave a message to schedule mandatory computer access and training.
