Inpatient Diabetes Care
CHRISTUS Santa Rosa New Braunfels offers expert inpatient diabetes care to hospitalized patients and their families. Our family nurse practitioner specializes in diabetes management and is available to assist in improving blood glucose control during hospitalization.
Individuals receive one on one educational sessions from our certified diabetes educators, dietitians and knowledgeable registered nurses. Newly diagnosed patients as well as those with high risk issues such as non-healing wounds, cardiac disease and episodes of low blood glucose are identified for evaluation and assistance. Our patient education channel offers a variety of programs on diabetes management and provides an opportunity for learning at any hour of the day or night.
Our team includes a certified diabetes educator who is also a certified insulin pump trainer who can assist patients with insulin pump therapy or those individuals with other insulin therapy needs.
Outpatient Diabetes Care
From intensive management with our diabetes nurse practitioner to specialized education in our American Diabetes Association recognized program, CHRISTUS Santa Rosa Hospital – New Braunfels offers a variety of services for the person with diabetes.
Management Services
Consultation with our diabetes nurse practitioner is available to those seeking intensive diabetes management. Patients receive a comprehensive approach to managing their disease in coordination with their primary care physician.
Interested individuals must have a referral from their primary care physician.
Individual Education
Our certified diabetes educator assists patients to manage their diabetes effectively using their meals, medications, activity, blood glucose monitoring and more. Our educator is also a certified insulin pump trainer and assists patients and their physicians with all aspects of insulin pump therapy.
Group Classes
Classes are held every month. Evening and day offerings are available. Participants receive instruction and direction from a team of dedicated diabetes professionals.
Our program is ADA recognized and regularly receives commendations from members of the community who now manage their diabetes successfully.
